FIELD	TITLE	Description
1	Date	Date specified for Significant Controller Register (document date).
2	RLE	Current name of Relevant Legal Entity and registered (default) address (for example, registered office).
2	Present Name	Current name of Individual and Usual Residential Address (URA).
3	Date Registrable	Earliest Date From of Significant Control records created for the Significant Controller/RLE.
	Date Entered	Latest Date of Entry on Significant Controllers Register for the Significant Controller/RLE.
	Date Ceased	Most recent Date To -Where no current Significant Control records/conditions exist for Significant Controller.
4	Legal Form	Company/Entity Type
	Registration Number/ Equivalent	Company/Other Entity registration number
	Governing Law	Other Entities Only. User Defined Field with "SYSGOVLAW" Quick Reference. Field may already be visible for use in UK (if both UK and HK entities exist). If not, to make visible: System Tools > User Defined Fields > All People
5	Nature of Control	Source of Conditions: Data Library, Significant Control
6	Designated Representative	Individual appointed as Designated Representative, with recorded Job Title (Capacity).
7	Contact Details	Work address and contact details for Designated Representative.

1	Former Surname Former Forename (If any)	Surname and forename entered into Previous Name field under Statutory section of Individual Details.
	Alias (if any)	All People User Defined Field with "ALIASNAME" Quick Reference. To make visible: System Tools > User Defined Fields > All People System Tools > User Defined Fields > All People
	Identity Card Passport Number	Standard fields under Individual Details. Displays ID Card detail by default. Will only display passport number if ID Card field is blank.
2	Issuing Country of Passport (if applicable)		Individuals Only User Defined Field with "PASSPORTCTRY" Quick Reference.
