Forms

You can use Policy Deploy forms to create attestation questionnaires.

Create a questionnaire

  1. From Policy Deploy, select Administration then Forms. You can copy an existing policy by selecting the button and edit the form. Or follow these steps to create a form:

    1. Select the Create Questionnaires button.

    2. Enter a Name (cannot contain spaces) and Description.

    3. Select the Form Type (such as Attestation) and Category (such as Attestation).

    4. Select Save. Continue to edit the questionnaire you just created.

Edit a questionnaire

You may edit a questionnaire. To see previous edits, select Show Version History.

  1. From Policy Deploy, select Administration then Forms.

  2. Select the form to edit.

  3. In the Questionnaire pane, verify the Name, Description, Questionnaire Type such as Attestation Form, and Questionnaire Category such as Attestation Form. These fields are not use: Category Sequence Number and Allow Multiple.

  4. In the Versions pane:

    1. Version is the version number being viewed. The version defaults to the most recent or active version.

    2. Language determines in which language the form will display. One form can display in multiple languages. Your organization must provide each translation.

    3. Active is select to make the form available for use. Once a form is activated, you cannot edit the version of the form.

    4. Description of the version being viewed.

  5. In the Version Question Listing pane, add questions, which may be simple text or may require a response. Questions can only be vertically stacked, not side-by-side. Select buttons to perform the following, as needed.

  6. Select Add Question to add a question requiring a response or just display text.

  7. Select Type of response then select the Select Type button. Once you identify the Type, it cannot be changed and, instead, you will need to delete the question.

    Give each Type a Name which is the short name of the question and does not display on the form. You cannot enter spaces or special characters, so using camel case is helpful such as textboxName. The editor is available to enter content. See Using the editing toolbar.

    Types used in Policy Deploy include the following. Other Types are not applicable.

    • Label: Enter text or a placeholder for images. A label does not ask a question.

    • Textbox: Creates a textbox for the users to type in text.

      • Select Required if the user must respond and then enter a Validation Message.

      • Text can be Single Line, Multi Line, or a Date. If single or multi line, identify the default number of Visible Columns and Visible Rows in the text box.

      • Prefill Options will fill content from the system.

    • Radio Buttons: Only one value can be selected. Reference Codes typically display. Select Required if the user must respond and then enter a Validation Message.

      For Select the Droplist Source, the best practice is to not select Specify explicit values and instead use Get values from RefCodes, Get values from other Fields, or Dependent list of values.

    • Check Boxes: Multiple values can be selected. Reference Codes typically display.

    • Dropdown List: One value can be selected from a dropdown list. Reference Codes typically display.

  8. In the Dependencies pane, Add or Delete the Parent and Child questions. The questions that depend on answers to other questions are identified. You can also select Mark for Deletion then Delete to remove dependencies.
  9. Select Save Form.
