The Matrix question type
This topic includes details about Matrix questions, which ask respondents to evaluate one or more row items using the same set of column choices.

From the Questionnaire Builder, select Add question.

Select Matrix question from the list of options, and then select "Add".

A question editor dialog appears. If you would like the Matrix question to display a title, type it into the Enter title (optional) field.

Type the question into the Enter text field. You can also copy and paste text into the text editor.

Enter text for the Rows. The row labels are the statements you would like respondents to rate. They appear on the left side of the Matrix table.
Note
You can add up to 100 rows and five columns in the Matrix table.

You have the option to apply Boolean logic. Boolean logic means a result can only have one of two possible values. Select the checkbox next to Boolean Question to apply Boolean logic.
Note
If you enable the Boolean Question checkbox, the number of columns and weights update automatically.

Enter text for the Columns. The column labels are the options you want respondents to select from. They appear on top of the Matrix table.

To update the weight of each column, you can select the Weights field, and then enter a number between one and 100.
Note
If you applied Boolean logic, a weight of one for positive options and zero for negative options appears in the Weights fields.

To preview the Matrix table, select Preview Matrix.

Select Save to add the Matrix question to the questionnaire.
