Creating a questionnaire
You create a questionnaire using a template. The Minimum Required Questions template is available as a base. A questionnaire can have system-supplied questions and up to 108 you create (identified with the genQuest identifier). There are a variety of types of questions you can add. See Question Types. Once you create a questionnaire, you cannot delete it but it can be inactive.
Create the questionnaire then add the questions and question dependencies.
- Select Settings then Content Control.
- Select Form A form is a questionnaire to gather information. Builder.
- Select the Create Form button.
- Complete the following:
- Enter a unique Name. If duplicate names are used, an error will occur when the respondent tries to open the questionnaire.
- If the questionnaire is a DDQ Due Diligence Questionnaire which can be sent to third parties for response. Questions can be added to a risk model to use in determining the overall risk rating for a third party., the name must not exceed 50 characters and may only contain the following characters:
Letters A through Z (upper or lower case Assigned to a third party profile related to a questionnaire sent for response, due diligence investigation, or other action.)

Spaces
Parenthesis
- Enter a Description to provide detail about the content, audience, use, or other information.
- Select a Template. For example, to start with a blank questionnaire, select Minimum Required Questions. The Logo question is included for you to add your organization's logo.

Do not reorder the questions (labels, textbox, and so on).
- Select a form Category which cannot be changed once selected. You may choose:
- External forms are requested by invitation to the third-party Point of Contact (POC). These questionnaires can be scored for risk directly, as long as their answers are yes/no or multiple choice. External forms can also have yes/no answers red-flagged for unexpected responses. You can only send one external form at a time.
- Internal forms are typically completed via a self-service portal (also called OpenURL) where business sponsors can create their own accounts, and then submit a new form for every relationship in scope. These internal forms cannot be scored directly. Instead, third-party custom fields Custom fields can be created to use in risk models. and custom lists are built that can then be included in the form and scored as part of the risk of the 3P Profile. See Setting fields and lists.
- Training forms can be sent to Associated Persons like key owners and managers (typically entered by the 3P POC on the DDQ). Where DDQs can only be sent to one person, training forms can be sent to multiple people at the same time.
- External forms are requested by invitation to the third-party Point of Contact (POC). These questionnaires can be scored for risk directly, as long as their answers are yes/no or multiple choice. External forms can also have yes/no answers red-flagged for unexpected responses. You can only send one external form at a time.
- Select the Sub-Category based on the type of form you are creating. Once selected, this cannot be changed.
The Sub-Category determines how many tabs will be displayed in the Case Folder. For example, L-85 has two tabs which are Company and Authorization. L-33 has six tabs which include the following. Select the Sub-Category with the content you want to capture in the form. (Some may be duplicates.)Company

- If the questionnaire is in the Category External, you may select Is Renewal to identify the questionnaire the new questionnaire is a renewal of. Select the form and form version the renewal is for. This way, the initial questionnaire has all questions and the renewal questionnaire may have fewer questions and may include a question like: Did anything change since last submitting?In a future release of forms 2.0, if renewal functionality is turned on, then renewDays must be set to 0.
Important
You can add questions to a renewal form or a new version of an existing form. Do not repurpose any existing questionID.
